Family visa for long-term visiting families(spouse has work permit in China)

4.Applicant’s Local Shanghai Registration Form of Temporary Residence(up to date)
5.Health Certificate of adult applicant(kids less than 18 years or parents over 70 years old are not necessary to provide the health certificate)
